Online EASI Form is Easy

To give EAP providers an easier way to submit client encounter documentation (as required for payment), Magellan has launched a new online version of our Employee Assistance Service Information (EASI) Form. The information mirrors the hard copy form to which you are accustomed, but can be conveniently completed and submitted entirely online. 

If you serve Magellan EAP clients, you now can complete the online version of the EASI Form directly on our provider website instead of mailing or faxing in forms.

You can use the online EASI Form for all EAP sessions with the exception of sessions provided under the United States Postal Service and Federal Occupational Health EAPs.

You also can view an EASI Form online training demo that shows how to use the new Web application. (Demos are available on the provider website under the top-menu item Education, then Online Training.)
