The Care Manager will be responsible for the assessment, reassessment, care planning and coordination of care and services including ongoing monitoring of an appropriate and effective person centered care plan; member education and care management. He/she will regularly communicate with the member’s PCP and other providers and integrate the member, caregiver and other provider feedback into the assessment and planning.

The Director, Health Services is responsible for the overall oversight/administration of daily activities for all clinical services and activities including Medical Management, Care Management (CM) Pharmacy Services Management (PSM), Utilization Data Analysis, and Utilization Management. Director of Marketing view details

The Director of Marketing will develop and implement AlphaCare’s marketing and communications plans and strategies. The position will oversee the creation of campaigns, collateral materials, member education pieces, and other member communications that increase awareness, brand loyalty, and clinical adherence. The position will also be responsible for generating a pipeline of sales leads through the implementation of targeted B2B and B2C advertising, will provide marketing consultation to product development, and collaborate with cross-functional teams to lend marketing support to company-wide initiatives. The position requires 10 years of marketing and communications management experience and strong health care background with 5 years of Medicare Advantage marketing experience strongly preferred. Managed Medicaid or MLTC marketing experience is also a plus.

Community Relations Representative view details

The Community Relations Representative is responsible for the development of AlphaCare’s referral network, branding and overall positioning with providers and prospective members. He/she will develop and maintain relationships with health care providers, community-based organizations, the Local Department of Social Services, the New York City Human Resources Administration, faith-based organizations, local businesses, and key-decision makers. Intake Nurse Team Lead view details

Responsible for the coordination of clinical intake activities necessary to ensure AlphaCare of New York meets NY State DOH and other MLTC regulatory standards. Network Development and Provider Relations

IPA Network Account Manager view details

Develops and refines strong business relationships with large network providers including IPA’s , large PC’s or hospital based physician practices .network administrators and internal business partners leading to the achievement of provider satisfaction, medical cost targets, network growth and/or efficiency targets. Educates providers and administrators on a variety of financial arrangements including shared risk as well as gainshare agreements and achievement of performance based incentive programs. Collaborates cross-functionally and uses pertinent data and facts to identify and solve a range of problems within area of expertise. Proficient at setting priorities and re-calibrating priorities on an ongoing basis dependent on the needs of the business.

Director of Hospitals, Skilled Nursing and Rehabilitative Services view details

The Director of Hospitals and Skilled Nursing and Rehabilitative Services will provide supervision over all contracted facilities including the management of the managing relationships in existing and newly contracted nursing homes. He/she will assist the Network and Provider Relations team in outreach efforts and expansion of new business assuming a lead role in the implementation of the I-SNP model in new facilities. He/she will assure compliance with the organization policy and I-SNP Model of Care and collaborate with other departments, as required, on Medicare reports, submissions, program materials, quality initiatives and CMS requests and requirements. He/she will be responsible for physician contracting, network development and provider service functions for all existing and newly contracted hospitals.
